Job Overview

We are seeking a compassionate and motivated Community Support / Outreach Support Worker to provide essential support to individuals within their local community. The role involves helping individuals develop life skills, providing emotional support, and ensuring they can access the necessary resources and services to live independently.

Key Responsibilities

  • Provide person-centered support tailored to the unique needs of each individual.
  • Assist with daily living activities, including budgeting, shopping, and maintaining a home environment.
  • Support individuals in accessing education, employment, and training opportunities.
  • Encourage and facilitate social inclusion and community engagement.
  • Provide emotional and practical support to young people and vulnerable individuals.
  • Develop and implement support plans in collaboration with individuals and relevant professionals.
  • Ensure safeguarding policies and procedures are adhered to and effectively implemented.
  • Work collaboratively with external agencies, including local authorities and healthcare professionals.
  • Maintain accurate records and documentation in accordance with company policies.
  • Promote independence and self-confidence among service users.
  • Support individuals with accessing and attending appointments as needed.

Requirements & Qualifications

  • Previous experience in a support worker, outreach, or community-based role (preferred but not essential).
  • Experience with children and young people and willingness to undertake NVQ Level 3.
  • Understanding of safeguarding policies and best practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving skills and a proactive approach.
  • Willingness to work flexible hours, including evenings and weekends.
  • A valid driver’s license and access to a vehicle (preferred but not essential).
